- Following the completion of your activity or event, as part of the funding agreement, your organisation must provide an activity/event report to the Mental Health Council of Tasmania by COB 30 July 2021.
Make sure you keep a copy for your records.
Please report on the achievements and feedback of your event against your planned objectives/goals.
We encourage short evaluation survey results, photographs, newsletters, media reports and other relevant documentation to be included with your report.

If you organised multiple events/activities, please let us know how many sessions you ran, and the total number of participants across all sessions.
The Mental Health Council of Tasmania acknowledges that there are sometimes changes in the cost of materials or services provided. However, the grant must be used for the purposes outlined in the event proposal and specifically on the items detailed in the application budget.
Funds not expended on the items outlined in the event budget must be returned to the Mental Health Council of Tasmania unless otherwise agreed in writing by Mental Health Council of Tasmania (with another appropriate purpose for those funds agreed upon by both parties).
I declare that:
• all funding received was expended for the purposes of the event or activity and in accordance with the event or activity proposal, including any changes to the Event or budget approved in writing by Mental Health Council of Tasmania;
• the information provided in this financial statement is a true record for the period indicated; and
• all terms and conditions have been complied with.
This declaration must be certified on behalf of the Grantee by one of the following: the organisation's board, chief executive officer or an officer with authority to do so.
